edu/doernbecher/CDRC Eugene The Eugene campus of the Child Development and Rehabilitation Center provides interdisciplinary clinical services for persons with developmental disabilities and other special health care needs.
CDRC Eugene programs and services are provided at the Clinical Services Building on the University of Oregon campus.
Function/Duties of Position: The Community Health Worker serves as an important member of the healthcare team at the Eugene CDRC.
The Community Health Worker works in partnership with a multi-disciplinary team to directly support patients and families in addressing social determinant barriers and improving individual and community health.
The Community Health Worker assists the multi-disciplinary team to coordinate and provide holistic services for patients and families that will improve health outcomes, improve engagement with OHSU and community resources and empower families to fully participate in the health care system.
**Position is located in Eugene, Oregon** Required Qualifications: High school diploma or equivalent.
Two (2) years relevant work experience in a mission-driven organization with one of those years being work experience with underserved populations with complex medical and social needs.
Preferred experience in providing community outreach services.
Certification as Traditional Healthcare Worker (THW) required.
Traditional Health Worker types include Doula, Peer Support Specialist (PSS), Peer Wellness Specialist (PWS), Personal Health Navigator (PHN) and Community Health Worker (CHW).
Type preference may be determined by service need.
Fluency in written and spoken English.
Must be able to communicate effectively verbally and in writing.
BLS/CPR (to be obtained within 6 weeks of employment).
Excellent telephone communication skills.
Must possess a valid motor vehicle driver license in good standing and must be eligible to operate a motor vehicle owned by OHSU or other agency; or demonstrate the ability to access transportation to meet the requirements of the position.
Intermediate level skills in Microsoft Word, Excel, and Outlook.
Understanding of Social Determinants of Health and role of SDH in healthcare.
Ability to work collaboratively with a multi-disciplinary team of health care providers.
Customer service skills with the ability to interact professionally and effectively.
Time management skills with the ability to prioritize and schedule daily activities for the most efficient use of time.
Preferred Qualifications: Experience providing direct service, community outreach, and/or advocacy.
Experience working with an electronic health record; EPIC platform.
Interest and experience in program development, improvement, and assessment.
Experience in community outreach services providing health information, advocacy, social support, and assistance engaging with the health care system.
Knowledge of learning and behavior change principles.
Knowledge of community-based resources.
