Licensed Mental Health Therapist

Applegate Medical Associates is looking for a full time licensed mental health therapist (LCSW or LPC) to provide excellent integrated care to our primary care patients in our busy practice.
The ideal candidate would be responsible for providing initial evaluations, individual, group and family therapy, as well as chronic disease management.
Therapist will be expected to support and have scheduled consultations with primary care and psychiatric providers to ensure our patients are receiving high quality integrated care.
