Churchill Estates is in search of a "Rock Star" LPN to join our team in providing professional, compassionate oversight of the care for our Assisted Living residents.

*LPN Job responsibilities:*
* Oversight of resident care according to their individual needs.
* Partners with Administrator and RCC to complete resident Care Plans and facilitate conferences with residents and involved families.
* Actively participate in Quality Assessment and Performance Improvement
* Provides training, mentoring and coaching for Medication Technicians and Caregivers so as to ensure optimal resident care and outcomes.
* Communicates and coordinates needs with outside Providers (PCP's, Hospice, Home Health, Specialists, etc.
) so as residents receive a fluid and sustainable continuity of care.
* Partners with RN in monitoring resident conditions and ensures proper systems and procedures are in place and are being followed accordingly.
* Coordinates outside care services with Hospice, Home Health, visiting PCP's, etc.
* Builds a positive, professional and caring relationship with the residents, involved families and outside agencies.
* Communicates residents needs to RN and Facility Administrator, in a timely and concise manner both verbally and by maintaining timely documentation and follow-up in resident records.
* Assists with investigations, implementation, and follow-up requirements for any regulatory concerns and issues.
* Steps in to administer medications as needed.
* Other duties as assigned.
*LPN Qualifications/Skills:*
* Excellent medical knowledge, including medical terminology.

*Education and Experience Requirements:*
* Completion of state approved training program (generally in form of certificate, diploma or associate degree)
* LPN License (pass NCLEX-PN exam) in good standing for Oregon
* BLS Certification
* 1-2 years recent LPN, or similar nursing experience preferred.
