POSITION SUMMARY: A Supported Living Provider is a life helper to individuals who possess a variety of abilities.
Our clients need some assistance to engage in community activities such as shopping, going on outings or getting a workout at the gym.
A Supported Living Provider will also assist individuals in their homes, mentoring basic life skills such as keeping a clean house, managing finances and dealing with situations that enter people's daily lives.
Providing this type of human service is very rewarding knowing you are truly making a difference in the quality of someone's life.
This position is less than 30 hours per week.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Transport clients and assist with errands and appointments such as grocery shopping, banking and medical appointments
Assist with order and pick up of prescriptions, understanding medication instructions, filling med boxes and scheduling medication reminders
Assist with accessing community resources such as food, housing and family services
Assist as needed with applications for various services
Provide support for community inclusion including hobbies, exercise, art and music classes, social groups, outings, and other preferred activities
Provide hands-on assistance and reminders with household chores and responsibilities, such as cooking, cleaning and life organization
Assist with activities of daily living such as hygiene and personal care, offering physical support as needed (transfer, ambulation, or standby assistance)
OTHER DUTIES AND RESPONSIBILITIES:
Maintain all necessary client records and record daily data entries into appropriate record keeping systems
Cross training in other areas including Job Coaching
MINIMUM QUALIFICATIONS AND EXPERIENCE:
High School Diploma or equivalent
Dependable transportation; valid Oregon driver's license with driving record sufficient to qualify to drive agency vehicles
Ability to pass an Oregon Department of Human Services background check
Experience working with adults with intellectual/developmental disabilities preferred
